   Look at attatchment, I got a letter for 4 visits during the same day June 2 this year and hope its a follow up to my Breathing, Hearin, foot, and Phsycological problems I saw all above but foot dr’s in 2012 but nobody in 1996 the first time. HOping this pans out as I was in Critical care 72 hrs 2 weeks back, and am now getting pain injections and nerve blockers…. Also expectiong poss one visit is for my headaches and neck pain I hope your letter has pushed this to a rating decision upon proof the problems exist and if not I am Sure it helped get another review…. Thanks and I appreciate the time it took you to get that done
